Include a form, such as a sign up form, poll, or similar in a page.
create a new page and embed the macro + then search for easy form
name your form and choose settings - you can enforce one response per user (based on login) and let users edit their responses.
save the page and there's your form. click edit form and start adding form fields like check boxes, drop-downs etc.
